The Investigation Guide has been developed to support you to meaningfully involve patients and families in patient safety incident investigations and meet the requirements of the Patient Safety Incident Response Framework (PSIRF).

It is important that you familiarise yourself with the Patient and Family Guide which works together with this, and signpost patients and families to the relevant sections. You might find that they come to the investigation process with clearer expectations about how they will be involved.

Both guides have been co-designed by a community of stakeholders including patients and their families, patient safety managers, investigators, healthcare staff and legal representatives, and supported by policy makers.
